I designed a flexible Form Builder that helped project coordinators create and customize forms for clinical trials. This tool streamlined workflows for data collection and patient tracking, improving efficiency across various clinical trial phases.

The Problem
Project coordinators needed a tool to manage reusable forms for different clinical workflows. The goal was to simplify data collection and make forms easily customizable to meet the unique needs of each study, while ensuring consistency and compliance across sites.
This project became key to improving form creation and productivity and is still in active testing to support ongoing needs.
Key Requirements

Form Variety
Support for various clinical trial documents, including startup forms, follow-up letters, CRFs, protocol uploads, and other study-specific forms.
Custom Templates
Allow modification of global templates to fit each study’s unique requirements.
Digital Transition
Convert paper-based forms into digital templates quickly, preserving their structure and functionality.
Design Process
In collaboration with project managers and developers, I conducted research on existing form-building tools and mapped out essential workflows. This helped me identify key design needs:
• Clear Structure: Organize forms in a section-row-field hierarchy for easy navigation.
• Accessible Settings: Field-level settings should be easy to access without interrupting the form-building process.
• Performance: Anticipate challenges with longer forms, ensuring the system would remain efficient even with a high number of fields.

The Solution
The final interface was composed of several modular sections:
Flexible Layout
A section-row-field hierarchy for better organization.
Metadata Panel
A static side panel for configuring field rules and metadata.
Stepper Toggle
Included an optional step-by-step switch to break long forms into more manageable pages, improving usability for users navigating longer forms.

Reflection
This project reinforced the importance of designing for scalability and flexibility. By anticipating future needs and thinking modularly, I was able to create a solution that could easily evolve with minimal rework. Working within a lean development team also taught me to make the most of our existing design system, balancing flexibility with the efficiency of development.
